As a field-based role, you will travel across the Dorset region and work from home. You will be part of a team delivering high-quality, client-centred care while ensuring regulatory compliance standards are met.

The ideal candidate has extensive experience working in Intensive Care Units (ICU), spinal care units, or with complex care patients. They should possess comprehensive knowledge of age-specific care practices, treatment plans, and communication strategies for effective patient management. The salary for this role is £30-£35 per hour, depending on the shift.

Key responsibilities include:

  • Supporting the safe transition and mobilisation of complex packages from hospital to home
  • Maintaining services for existing clients, ensuring ongoing high standards of care and safety
  • Providing clinical training to support workers and collaborating with Complex Care Managers
  • Liaising with clients and support workers to review care packages and implement timely remediations
