Job Overview

We are currently seeking a Home Support Specialist to join our team. This role involves providing one-to-one support to our clients in their own homes and during community activities. As a Home Support Specialist, you will work closely with our clients to understand their unique needs and develop personalized care plans.

Key Responsibilities
  • To provide high-quality support to clients with varying levels of need, ensuring their safety, dignity, and well-being at all times.
  • To develop and implement personalized care plans in collaboration with clients, family members, and other relevant stakeholders.
  • To maintain accurate and detailed case records, including risk assessments, positive behavior plans, communication passports, and sensory arousal ladders.
Requirements

To be successful in this role, you will need to possess excellent interpersonal skills, be able to work independently, and demonstrate a genuine commitment to delivering high-quality care. You will also require the ability to write accurate and detailed case records and contribute to client care plans.
