About the Role:

We are seeking an experienced Senior Rehabilitation Specialist to join our team in Milton Keynes. As a key member of our multidisciplinary team, you will work closely with service users, families, and other healthcare professionals to deliver high-quality rehabilitation services.

Key Responsibilities:

  • Develop and implement individualised care plans, addressing the unique needs of service users.
  • Collaborate with multidisciplinary teams to ensure seamless transitions and effective communication.
  • Conduct thorough risk assessments, identifying potential hazards and implementing strategies to mitigate them.
  • Provide guidance and support to rehabilitation support workers, promoting best practice and continuous learning.
  • Stay up-to-date with industry developments, attending relevant training and conferences to enhance your skills and expertise.

What We Offer:
