Main Purpose of the Role

As a member of the Local Solutions team, you will work with unpaid carers to support them in accessing practical and emotional support. Your role will be to empower carers to take control of their own health and wellbeing, focusing on the 'what matters to me' philosophy and taking a holistic approach to connect carers to community groups and statutory services.

Key Responsibilities
  • To support carers through the Carers Assessment process, using a variety of methods such as home visits or community-based support.
  • To complete Carers Support Plans with carers, using a person-centred approach.
  • To provide carers with information on the range of services available and how to access them.
  • To complete and submit carers support plans in a timely manner, ensuring that departmental KPIs are achieved.
  • To liaise with colleagues in statutory services and develop relationships that support unpaid carers.
Accountabilities
  • To develop and sustain links with partnership organisations to encourage collaborative working to support unpaid carers.
  • To provide advice, guidance, and practical support in the implementation of support packages.
  • To support the collation, analysis, and reporting on opportunities for organisational learning on Carers projects.
Responsibilities
  • To work collaboratively with key stakeholders to raise awareness of carers known to them.
  • To promote the referral pathway for carers.
  • To support carers to access relevant support dependent on their needs.
  • To participate in presentations, attend meetings, and provide information at events and activities that offer support to carers.
  • To provide monthly KPIs and reports as required.

  • To ensure that all activities of the project provide equality of opportunity.
  • To ensure that all policies and procedures are implemented within your working practice.
Requirements
  • Experience of working in health or social care.
  • Experience of supporting unpaid carers or working with people.
  • Experience of working as part of a team, prioritising own workload, managing own diary, building relationships with carers and professionals, and using management databases.
  • Attendance at occasional evening or weekend events.
  • Technical expertise in using Microsoft Office and Outlook.
  • An enhanced Disclosure & Barring Check and mandatory safeguarding training as appropriate for the role.
  • A car owner/driver is essential for this city-wide post.
