Job Title: Carers Development Worker

Job Summary:

We are seeking a highly motivated and experienced Carers Development Worker to join our team at Local Solutions. As a key member of our team, you will work closely with unpaid carers to provide support, guidance, and practical assistance to help them access the resources they need to maintain their health and wellbeing.

Key Responsibilities:

  • Support carers through the Carers Assessment process, providing guidance and assistance to help them access the services they need.
  • Develop and implement Carers Support Plans, working closely with carers to identify their needs and goals.
  • Provide carers with information about the range of services available to support them, and help them access these services.
  • Liaise with colleagues in statutory services to ensure seamless support for carers.
  • Work collaboratively with key stakeholders to raise awareness of carers and promote the referral pathway for carers.
  • Support carers to access relevant support services, and provide regular updates and reports as required.

Requirements:

  • Experience of working in health or social care, with a focus on supporting unpaid carers.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with carers and professionals.
  • Ability to work independently and as part of a team, with a strong focus on prioritizing tasks and managing workload.
  • Technical expertise in Microsoft Office and Outlook, with the ability to use databases and input information accurately.
  • Enhanced Disclosure & Barring Check and mandatory safeguarding training as required for the role.
  • Travel is required, and a car owner/driver is essential for this city-wide post.
