Children and Young People's Wellbeing Practitioner

Cranstoun are seeking a dedicated and experienced practitioner to join our team in Oxfordshire, working with children and young people who are struggling with substance misuse.
You will work with a range of partner agencies to provide a holistic service that meets the needs of young people and their families, using a trauma-informed approach to specialist assessments, care planning, and interventions.
Your Key Responsibilities- You will hold and manage a caseload of children & young people, working with them to identify their needs and develop a personalized plan to address their substance misuse issues.
- You will provide specialist assessments, care planning, and deliver evidence-based psychosocial brief and structured interventions through one-to-one appointments, groups, and online.
- You will work closely with partner agencies to provide a comprehensive service that addresses the complex needs of young people and their families.
- It is essential that the successful candidate has a full driving license and can pass an Enhanced DBS Disclosure.
- The estimated salary for this role is £34,000 - £40,000 per annum, depending on experience.
