Key Responsibilities
  • Develop and implement effective support plans to facilitate clients' health, wellbeing, and independence.
  • Collaborate with service users to identify their strengths, goals, and aspirations, ensuring their active participation in co-produced support plans.
  • Foster strong relationships with professionals within the community to facilitate effective liaison and advocacy.
  • Work collaboratively as part of a dynamic team to ensure seamless service coverage and meet contractual obligations.

The ideal candidate will possess excellent communication skills, both written and verbal, with a good understanding of MS Excel and Word. They should also demonstrate a flexible and proactive approach to work, along with experience or awareness of supporting people with mental health and complex needs.
