About the Role

We are looking for a highly skilled Care Co-ordinator / Scheduler to join our team in supporting young adults with neurological disorders and learning disabilities in the local community.

Job Overview

This is a full-time position requiring previous experience in the care sector, ideally as a Care Co-ordinator / Care Scheduler in a domiciliary care environment. You will be based within a 15-mile radius of Crawley and must have access to a car for work purposes.

Key Responsibilities:
  • Rota and Schedule Management: Prepare rotas and schedules accounting for travel time, holidays, training, and last-minute changes or cancellations.
  • Care Team Facilitation: Ensure smooth and efficient communication between clients and their care teams, ensuring timely completion of required tasks and actions.
  • Staffing Collaboration: Work with the Registered Manager to maintain adequate staffing levels for both current and future needs.
  • Care Package Management: Collaborate with the Care Team to ensure new and existing care packages are appropriately resourced and scheduled in a timely manner.
  • Liaison and Communication: Act as the liaison between clients, care teams, and third-parties (e.g., GP practices), keeping all parties informed of any changes to client care requirements.
