Practice Nurse Specialist

Job Summary
The Practice Nurse will play a vital role in providing high-quality nursing care to patients at Southview Medical Practice. This is an exciting opportunity for a skilled and experienced nurse to join our team and contribute to the delivery of excellent patient care.
Main Responsibilities
• Provide comprehensive nursing assessments and develop care plans in conjunction with GPs and other healthcare professionals.
• Work closely with Health Care Assistants to develop their skills and support the practice in achieving quality indicators and targets.
• Perform basic nursing duties, including venepuncture, ECG, wound care, and cervical screening.
• Undertake accredited training in specific chronic disease management, such as diabetes, respiratory, and cardiovascular disease.
• Contribute to the maintenance of practice disease registers and provide advice on lifestyle issues.

Person Specification
• Registered General Nurse with evidence of ongoing post-basic education at diploma or degree level.
• Competent in basic nursing duties required for the post.

• Willingness to undertake further training and personal development as identified by the practice.
