This is a part-time permanent position with a shift pattern of Monday to Friday, covering 29.6 hours per week. Please note that the salary is pro rata for part-time hours.

We are seeking a compassionate and dedicated nurse to join our Focused Intervention Team (FIT) within West Dunbartonshire Health and Social Care Partnership as a Community Staff Nurse (Band 5).

About the Role

As a Band 5 Nurse within FIT, you will be responsible for managing a caseload of patients, working closely with the team to ensure individuals can safely remain at home, avoiding unnecessary hospital admissions. You will play a key role in delivering high-quality care, supporting the patient's recovery and independence in their own home environment.

Key Responsibilities
  • Manage a caseload of patients, providing care and interventions to enable them to stay at home as a safe alternative to hospital care.
  • Collaborate with the COPD Nurse (Band 6) and Care Home Liaison Nurse (Band 6), offering support when necessary.
  • Provide cover for the Band 6 nurses during absences or periods of high workload to ensure seamless care delivery.
  • Contribute to multidisciplinary team working, ensuring integrated care for patients.
Essential Skills and Qualifications
  • Registered Nurse with a valid NMC Pin.
  • Experience in a community nursing setting or similar role is desirable.
  • Strong clinical decision-making and assessment skills.
  • Ability to work independently and as part of a multidisciplinary team.

The estimated annual salary for this role is £32,300 - £43,041, depending on experience and location.
