About the Role

We are seeking a highly skilled and compassionate Rehabilitation Case Manager to join our dynamic team of healthcare professionals at Everpool.

Key Responsibilities
  • Collaborate with multidisciplinary teams to assess and develop individualized rehabilitation plans for patients.
  • Conduct thorough assessments of patient needs and monitor their progress throughout the rehabilitation process.
  • Complete immediate needs assessments and recommendations of care.
  • Liaise with external healthcare providers and insurance companies to coordinate and facilitate patient care.
  • Provide ongoing support, education, and counseling to patients and their families.
  • Maintain accurate and up-to-date patient records and documentation.
  • Stay current with industry trends and research to ensure the highest quality of care.
Requirements
  • HCPC or NMC qualification is mandatory.
  • Strong communication, interpersonal, and problem-solving skills are a must.
  • Excellent report writing and documentation skills.
  • Vast knowledge of acquired brain injury, brain injury in adult care, with a variety of other condition knowledge, including spinal injury, amputations, and pediatrics.
  • Proven experience as a Case Manager or in a related clinical role is extremely advantageous.
