Rehabilitation Case Manager Opportunity

We are seeking a highly skilled and compassionate Rehabilitation Case Manager to join our dynamic team of healthcare professionals at Everpool Recruitment.

This role offers genuine flexible working and is remote with field-based commuting, with expenses and occasional stay overs, paid.

Key Responsibilities:

  1. Collaborate with multi-disciplinary teams to assess and develop individualised rehabilitation plans for patients.
  2. Conduct thorough assessments of patient needs and monitor their progress throughout the rehabilitation process.
  3. Complete immediate needs assessments and recommendations of care.
  4. Liaise with external healthcare providers and insurance companies to coordinate and facilitate patient care.
  5. Provide ongoing support, education, and counselling to patients and their families.
  6. Stay current with industry trends and research to ensure the highest quality of care.

Requirements:

  1. HCPC or NMC qualification is mandatory along with a full driving licence.
  2. Excellent report writing and documentation skills.
  3. Vast knowledge of Acquired brain injury, Brain injury in adult care, with a variety of other condition knowledge, including Spinal injury, Amputations, and paediatrics.
  4. You must have worked with patients in a rehabilitation/complex care setting.
