About the Role:

We are seeking a dedicated and compassionate Care and Support Professional to join our team in Beaconsfield. As a Support and Development Worker, you will play a vital role in promoting and delivering a person-centred support culture across our service.

Key Responsibilities:

  • Support individuals with all aspects of their daily living, including personal care and medication management.
  • Coach and support individuals to learn independent living skills, such as cooking and laundry tasks.
  • Enable individuals to access the community for social, educational, or employment opportunities.
  • Safeguard service users' rights and dignity.
  • Contribute to the development and implementation of support plans and risk assessments.
  • Communicate effectively with the team and attend regular team and service user meetings.
  • Liaise professionally with family members and other professionals.
