About the Role:

Unique Training Solutions is seeking a highly motivated and compassionate Care and Support Worker to join our team at Holt House, a residential care home in Prestwich. As a Care and Support Worker, you will play a vital role in delivering excellent person-centred care to our residents, ensuring their physical, emotional, and social needs are met.

Key Responsibilities:

  • Contribute to person-centred assessment planning, implementation, and review to ensure residents receive tailored care.
  • Deliver excellent person-centred care, recognising the unique needs of residents with dementia.
  • Fulfill the role of key worker for allocated residents, building strong relationships and providing emotional support.
  • Support independence in daily living tasks, empowering residents to maintain their autonomy.
  • Promote equality, diversity, and inclusion, ensuring a welcoming environment for all residents.

Providing Care:

  • Share in person-centred assessment planning, implementation, and review to ensure residents receive comprehensive care.
  • Recognise, respect, and support the social, emotional, and identity needs of residents, delivering activities that cater to their interests.
  • Remain alert and attentive to residents' needs, ensuring their requirements are met throughout the shift.
  • Enable interaction and communication, adapting to the unique needs of residents with dementia.
  • Ensure actions support the care, protection, and well-being of residents, maintaining a safe environment.
  • Help address physical comfort needs, supporting residents to manage their pain and discomfort.
  • Support residents with personal care needs, including continence management.
  • Provide support for mobility, contributing to moving and handling residents with care.
  • Support residents to monitor their own health care needs, including medication use and administration.
  • Support residents in journeys to and from the home, ensuring their safety and well-being.
  • Engage residents in recreational and development activities, fostering a sense of community and belonging.
  • Provide food and drink to residents, assisting them in eating and drinking if necessary.
  • Respond to residents' refreshment requests, ensuring their needs are met in accordance with their care plans.
  • Support residents who are distressed, maintaining a respectful and caring attitude.
  • Support residents at the end of their life, providing comfort and dignity.

Recording:

  • Observe, monitor, and record residents' conditions, reporting as required and communicating with colleagues.
  • Complete records for residents, contributing to care plans and ensuring accurate documentation.

General:

  • Support the health and safety of yourself and residents, adhering to relevant legislation and Unique Training Solutions procedures.
  • Attend and participate in staff meetings, contributing to team discussions and decision-making.
  • Participate in supervision and appraisal, ensuring ongoing professional development.
  • Share in a flexible rolling rota, adapting to changing care needs and schedules.
  • Develop your knowledge and practice through training provided, enhancing your skills and confidence.
  • Follow the guidance set out in the Code of Conduct for Healthcare Support Workers and Adult Social Care Workers in England.
  • Work positively as a team member, maintaining effective and harmonious relationships with colleagues.
