About the Role

We are seeking a highly motivated and enthusiastic Community Health Care Support Worker to join our Community Intensive Therapy Team within the Emotional Wellbeing and Mental Health Care Group at Cardiff and Vale University Health Board.

Key Responsibilities
  • Support children and young people with moderate to severe mental health difficulties in the community.
  • Work closely with the young person and their family or carer to offer community intervention and monitor mental health needs.
  • Collaborate with the wider CAMHS Multi-Disciplinary Team to provide comprehensive support.
  • Develop and maintain effective relationships with local authorities, education providers, and third sector organizations.
  • Contribute to the development and implementation of individualized care plans.
Requirements
  • NVQ III in Health Care qualification or relevant experience, knowledge, and training working with children and young people.
  • Experience of working with children and young people in a similar environment.
  • Awareness of child and adolescent normal development.
  • Excellent communication skills, ability to work within a team, and autonomously.
  • Ability to work flexible under close supervision.
Desirable Qualifications
  • Formal qualification in areas relevant to the post.
