We are seeking a highly motivated and enthusiastic Community Health Care Support Worker to join our Community Intensive Therapy Team within the Emotional Wellbeing and Mental Health care group.
The successful candidate will work closely with children and young people who present with complex, high-risk emotional wellbeing and mental health difficulties, providing support and intervention in the community.
As a key member of the team, you will work collaboratively with health colleagues, local authority, education, and 3rd sector partners to provide a comprehensive service to children and young people and their families/carers.
Key Responsibilities- Support children and young people with moderate to severe mental health difficulties in the community.
- Work closely with the young person and their family or carer to offer community intervention and monitor mental health needs.
- Collaborate with the wider CAMHS Multi-Disciplinary Team to provide a comprehensive service.
- Develop and maintain effective relationships with stakeholders, including health colleagues, local authority, education, and 3rd sector partners.
- NVQ III in Health Care qualification or relevant experience, knowledge, and training working with Children and Young People.
- Experience of working with Children and Young People in a similar environment.
- Awareness of Child and Adolescent normal development.
- Excellent communication skills, ability to work within a team and autonomously.
- Ability to work flexible under close supervision.
