Community Health Care Support Worker EWMH

About the Role
The post holder will work as part of the Community Intensive Therapy Team to support children and young people who present with complex, high-risk emotional wellbeing and mental health difficulties.
They will work with the wider multidisciplinary team to provide support to children and young people and their family/carer in the community during their time of distress.
They will work closely with the team including health colleagues, local authority, education, and 3rd sector.
Main Responsibilities
The post holder will be expected to support children and young people with moderate to severe mental health difficulties who may face challenges within the community such as tending to their educational and social needs.
The post holder will work closely with the young person and their family or carer to offer community intervention and monitor mental health needs whilst being supported by the wider CAMHS Multi-Disciplinary Team.
