Community Health Care Support Worker EWMH

We are seeking an enthusiastic and motivated Community Health Care Support Worker to join our Community Intensive Therapy Team within the Emotional Wellbeing and Mental Health care group.
The post holder will work as part of a multidisciplinary team to provide support to children and young people with moderate to severe mental health difficulties in the community.
Key responsibilities include:
- Providing community intervention and monitoring mental health needs
- Working closely with the young person and their family or carer
- Supporting children and young people with complex, high-risk emotional wellbeing and mental health difficulties
- Collaborating with health colleagues, local authority, education, and 3rd sector
We are looking for someone with excellent communication skills, ability to work within a team and autonomously, and experience of working with children and young people in a similar environment.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome.
