Key Responsibilities

  • Conducting desk-based and face-to-face assessments of referred individuals to identify their needs and develop personalized support plans.
  • Ensuring an APIE approach to assessment, planning, implementation, and evaluation.
  • Reviewing reports and evidence to inform clinical practice, business performance, and quality measures.
  • Identifying and mitigating complex risks in a community setting.
  • Producing clinically informed assessments with robust risk assessment and identifying risk mitigation measures.
  • Attending internal and external multidisciplinary team meetings to inform assessment and risk mitigation.
  • Building professional working relationships and effective time management.
  • Record keeping and maintaining accurate, safe, and contemporaneous records.

Essential Skills and Experience

  • Registered Occupational Therapist (OT) with experience in assessing and working with individuals with complex needs.
  • Understanding of continuing healthcare and transition of individuals from hospital/residential settings into the community.
  • Applied knowledge of the Mental Health Act, CTOs, and the Mental Capacity Act, Deprivation of Liberty Safeguards, and Best Interest Decision making.
  • Expertise in assessment and management of risk, with strong report writing skills and computer literacy.
  • Full UK driving license and access to a vehicle.
