Job Title: Specialist Community Nurse

Job Summary:

We are seeking an experienced and skilled Specialist Community Nurse to join our team at Wiltshire Health and Care LLP. As a Specialist Community Nurse, you will be responsible for delivering high-quality care to patients in their homes and community settings.

Main Responsibilities:

  • Provide expert care to patients with complex needs in their homes and community settings.
  • Work collaboratively with multidisciplinary teams to develop and implement care plans.
  • Conduct assessments and provide evidence-based interventions to improve patient outcomes.
  • Develop and maintain relationships with patients, families, and carers to ensure person-centred care.
  • Participate in quality improvement initiatives to enhance patient care and services.

Requirements:

  • Registered Nurse with a valid NMC registration.

  • Ability to work autonomously and as part of a team.
