Job Duties

- Clinical Practice Responsibilities:

  • Assess, plan, develop, implement, and evaluate health promotion programs aimed at enhancing well-being and preventing health issues.
  • Create and assess individualized treatment plans for patients with established chronic conditions.
  • Identify and manage treatment strategies for patients at risk of developing chronic conditions.
  • Prioritize health concerns and intervene effectively in complex or urgent situations, including initiating emergency care when necessary.
  • Review patient medications for therapeutic effectiveness, ensuring alignment with evidence-based practices and established protocols.
  • Collaborate with patients to support adherence to prescribed treatment regimens.
  • Encourage patients to adopt health promotion strategies that foster healthy living and self-care principles.
  • Utilize opportunities such as new patient assessments to deliver health promotion initiatives.
  • Provide guidance on prescribed and over-the-counter medications, including potential side effects and interactions.
  • Assess and manage uncomplicated wound care.
  • Offer support and information to women regarding family planning needs.
  • Address the health requirements of women undergoing cervical cytology consultations.
  • Recognize and refer patients with mental health concerns in accordance with national frameworks.
  • Participate in vaccination and immunization programs for both adults and children.
  • Advise and administer vaccinations for patients traveling abroad, as appropriate.
  • Deliver evidence-based care for patients with chronic conditions.

Qualifications

Essential

  • Registered General Nurse.
  • Current NMC registration.
  • Evidence of ongoing professional development.
  • Ability to work independently without direct supervision.

Desirable

  • Qualifications or experience in cervical cytology.
  • Experience in childhood immunization and travel vaccination.
  • Background in chronic disease management for one or more conditions.

Experience

Essential

  • Minimum of one year of experience in a general practice setting.
  • Familiarity with treatment room responsibilities required for this role.
  • Understanding of clinical governance in primary care.
  • Knowledge of patient group directions and related policies.
  • Experience with quality initiatives such as QOF and KPIs.

Desirable

  • Experience with clinical systems.
  • Awareness of local health issues.
  • Understanding of local and national health policies.
