Position Overview

An exceptional opportunity has emerged for a Clinical Nurse Specialist to become a vital member of our well-established Anticoagulation Service. This role is integral to the delivery of a comprehensive Warfarin monitoring and dosing service within our healthcare system.

Key Responsibilities

The successful candidate will be responsible for:

  • Delivering a commissioned Warfarin monitoring and dosing service, ensuring that patient treatments remain within therapeutic ranges.
  • Adjusting drug dosages as necessary and implementing timely interventions to enhance patient care.
  • Collaborating effectively with a multi-disciplinary team to provide safe and efficient healthcare services.
  • Managing patient interactions within a clinical environment, including addressing inquiries from patients, caregivers, general practitioners, and other healthcare professionals.
  • Supporting patients who are self-testing, promoting their independence and self-management of chronic conditions.

The role will also involve monitoring and managing patients across various locations, necessitating travel to outreach clinics as required. A valid driver's license and access to a vehicle are essential for this position.
