Job Summary: We are seeking a highly skilled and experienced Hospital Social Worker to join our team at Pertemps. As a key member of our Adult Social Care team, you will be responsible for providing high-quality social care services to individuals and their families.

Key Responsibilities:

  • Conduct comprehensive assessments of social care needs and risks.
  • Develop and implement individual care plans that promote dignity and empowerment.
  • Work collaboratively with multi-disciplinary teams to achieve optimum outcomes for service users.
  • Provide advice and support to unqualified staff and students to assist their development.

Requirements:

  • Qualified Social Worker registered with Social Work England.
  • SW, CSS, DipSW or Social Work Degree.
  • Experience in a Social Services Directorate or other Statutory or Voluntary Agency.
  • Excellent communication and mediation skills.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
