District Nurse

We are seeking a skilled and compassionate District Nurse to join our community nursing team at Newport Pagnell Medical Centre. As a District Nurse, you will play a vital role in providing high-quality nursing care to patients in their own homes, promoting independence and improving health outcomes.
Main Responsibilities:- Provide skilled nursing interventions to patients of all ages and conditions, working in partnership with the community nurse team and general practitioners.
- Undertake delegated duties under the supervision of the community nurse team leader or senior on duty.
- Maintain close liaison with the community nurse team leader, the team, GP, and other members of the primary care team and wider health and social care team.
- Apply current infection control policies and universal precautions to ensure patient safety.
- Encourage patients towards self-care and independence, promoting holistic and evidence-based practice.
- Maintain up-to-date knowledge of methods of rehabilitation and provide education to patients and their families.
- Ensure that adequate medical supplies, equipment, and other materials are maintained.
Newport Pagnell Medical Centre is a large GP training practice committed to providing excellent primary health care services with a strong emphasis on staff training and development. We have an 'outstanding' CQC rating and are dedicated to delivering high-quality care to our patients.
We are a large training practice based in the town of Newport Pagnell with another surgery in the Willen area. Our team is passionate about providing innovative and patient-centered care, and we are seeking a like-minded individual to join our community nursing team.
Person Specification:- Current NMC registration with an active PIN.
- Previous experience working as a practice or community nurse.
- Experience of working in a primary care environment.
- Experience of chronic disease management, infection control, and rehabilitation methods.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
