Job Summary

We are seeking a highly skilled and experienced District Nurse to join our team at Your Healthcare CIC. As a District Nurse, you will be responsible for providing high-quality nursing care to patients in their own homes and other community settings.

Key Responsibilities
  • Assess and plan the holistic health and social care needs of service users and carers through the Nursing Assessment Process.
  • Coordinate, agree, and implement care plans in conjunction with the District Nurse and other healthcare professionals.
  • Act as the Care Manager for an agreed number of service users with assessed nursing needs or who meet the Continuing Care Criteria.
  • Monitor and review care plans, evaluating services and making recommendations for improvement.
  • Develop expertise in risk assessment and management, ensuring the use of these skills in practice at all times.
  • Participate in health promotion activities and contribute to service development and clinical governance activities.
  • Provide leadership and direction to the nursing team in the absence of the District Nurse.
