We are seeking a talented Community Health Leader to join our team in Sefton. This role comes with an estimated salary of £55,000 - £62,105 per annum.

About the Opportunity

In this role, you will have the opportunity to lead and manage a District Nursing Team, driving service improvements and enhancing patient care. You will work closely with our Integrated Community Nursing Senior Leadership Team to develop effective strategies and promote a positive work environment.

Key Responsibilities
  • Lead and manage a District Nursing Team to achieve exceptional patient outcomes and satisfaction
  • Develop and implement evidence-based practice to improve patient care and services
  • Collaborate with multidisciplinary teams to ensure seamless care delivery and quality improvement
  • Ensure adherence to relevant policies, procedures, and guidelines
  • Provide expert clinical leadership and guidance to the team
Essential Qualifications and Experience
  • First-level Registered Nurse qualification
  • Nurse Prescriber V150/300 certification
  • Evidence of post-registration education in related areas
  • Proven experience in leadership and management roles within community nursing settings
  • Excellent communication and interpersonal skills
