About the Role

This is a challenging and rewarding opportunity for a skilled Community Health Specialist to join our multidisciplinary team.

As a District Nurse/Community Sister/Charge Nurse, you will be responsible for providing clinical case management for patients with complex health needs who are at risk of deterioration.

You will work closely with the team to coordinate and delegate individual patient care, ensuring the highest standards of care are delivered.

Key responsibilities include:

  • Case managing a designated caseload of patients and managing/delegating the delivery of care.
  • Supporting the team leader and clinical lead in processes for providing, monitoring, and improving the quality of care provided to patients.
  • Providing clinical supervision/peer review, active clinical leadership, and mentorship to junior colleagues.

This role requires a strong background in nursing, with a minimum of 2 years community experience and 4 years post-registration experience.

You will have a range of clinical skills suitable to community nursing and experience of Case Management.

The successful candidate will be registered with the Nursing and Midwifery Council and have a valid Disclosure and Barring Service (DBS) check.
