Job Title: Community Health Nurse

We are seeking a skilled and compassionate Community Health Nurse to join our multidisciplinary team in providing high-quality care to patients in our rural community.

About the Role

This is an exciting opportunity for a registered nurse to work in a dynamic and supportive environment, coordinating and delegating individual patient care across secondary and social care settings.

As a Community Health Nurse, you will be responsible for managing a designated caseload of patients with complex health needs, providing clinical case management, and collaborating with the team to ensure seamless care delivery.

Key Responsibilities
  • Case manage a designated caseload of patients with complex health needs
  • Provide clinical supervision and peer review to junior colleagues
  • Support the team leader and clinical lead in quality improvement initiatives
  • Act as a practice assessor for staff and students
Requirements
  • Registered General Nurse
  • Post-registration study
  • Case Management experience
  • Minimum 2 years community experience
