Job Summary

We are seeking a skilled and compassionate District Nurse/Community Sister/Charge Nurse to join our dynamic team at Sussex Community NHS Foundation Trust. As a key member of our multidisciplinary community team, you will be responsible for coordinating and delegating individual patient care across secondary and social care settings.

Main Responsibilities

  • Manage a designated caseload of patients and delegate the delivery of care by the integrated team
  • Support the team leader and clinical lead in processes for providing, monitoring, and improving the quality of care provided to patients
  • Have oversight of all clinical areas, facilitating junior staff to lead on specific areas
  • Deputise for the Clinical Nurse Lead as and when necessary
  • Provide clinical supervision/peer review, including active clinical leadership and mentorship to junior colleagues
  • Be aware of the Trust financial position and the implications on service delivery
