Job Title: Community Health Nurse

We are seeking a skilled and compassionate Community Health Nurse to join our dynamic team at Sussex Community NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality patient care and promoting health and wellbeing in our community.

Job Summary

The successful candidate will be responsible for managing a designated caseload of patients, providing clinical case management, and delegating the delivery of care by the integrated team. You will work closely with the team leader and clinical lead to ensure the quality of care provided to patients, including risk management, compliance with CQC requirements, and clinical audit.

Key Responsibilities
  • Case manage a designated caseload of patients and manage/delegate the delivery of care by the integrated team
  • Support the team leader and clinical lead in the processes for providing, monitoring, and improving the quality of care provided to patients
  • Provide clinical supervision/peer review, including active clinical leadership and mentorship to junior colleagues
  • Be aware of the Trust financial position and the implications on service delivery, including overseeing the management of clinical stock and maintenance of equipment in accordance with Trust policy
Person Specification

We are looking for a candidate with the following qualifications and experience:

  • Registered General Nurse
  • ENB 998/Mentorship Qualification
  • Post-registration study
  • Minimum 2 years community experience
  • 4 years post-registration experience
