Job Title: Support Practitioner - Adults with Learning Disabilities Specialist
About the Role:
We are seeking a skilled Support Practitioner to join our team at Highland Home Carers. As a Support Practitioner, you will be responsible for providing high-quality care and support to individuals with varying degrees of learning and physical disabilities. Your role will involve supporting individuals with personal tasks, such as personal care, medication, and meal preparation, as well as enabling them to enjoy a wide range of person-centered activities.
Key Responsibilities:
- Assist individuals with personal care and daily living tasks
- Support individuals with their prescribed medication and maintain records as required
- Ensure individuals' homes are kept clean and tidy and that household duties are kept up to date
- Support individuals to maintain their tenancy and report any issues to the relevant agency
- Respect confidentiality at all times and clearly explain HHC policies about confidentiality to individuals and other colleagues
Person Specification:
- Commitment to learning and keeping skills updated
- Excellent interpersonal skills; an effective and sensitive communicator
- Ability to communicate with individuals, their families, and a range of other health professionals
- Flexible approach to work and a willingness to embrace and commit to person-centred support
