Care Management Lead – Salary £14.89ph


Voyage Care Ltd offers a competitive pay rate of £14.89 per hour, which is in line with the national average for care management roles in the Norwich area.

We are seeking a highly motivated and experienced Care Management Lead to join our team in Swaffham & Kings Lynn. As a key member of our community service team, you will be responsible for managing a team of colleagues and providing high-quality support to individuals with autism, learning disabilities, complex needs, and challenging behaviours.

Key Responsibilities:

  • Develop and implement person-centred support plans
  • Conduct care needs assessments
  • Induct new colleagues
  • Complete supervisions and appraisals
  • Manage medication competencies and audits
  • Attend multi-disciplinary meetings
  • Complete reports

You will also participate in the on-call rota approximately one week in four. This role requires someone who is passionate about enhancing the quality of life of the people we support and has experience working in a care management role.
