Role Overview

**Band 7 Diabetes Specialist Nurse Opportunity**

This role involves working with primary care teams to develop and implement effective diabetes management plans, utilizing expertise to drive improved patient outcomes.

The successful candidate will possess excellent communication skills, enabling them to influence multidisciplinary teamwork and facilitate seamless care coordination.

Responsibilities and Requirements
  • Manage complex cases of diabetes, ensuring comprehensive care and promoting patient self-management.
  • Work closely with GP practices and other healthcare services to establish robust systems for diabetes care.
  • Promote evidence-based practice, adhering to national guidelines and service frameworks.

Requirements:

  • Degree-qualified nurse with HCPC registration and relevant postgraduate qualification in diabetes care.
  • Significant experience in diabetes specialist nursing, preferably within a similar trust or organization.
  • Proven ability to communicate effectively with diverse stakeholders, influencing multidisciplinary teams and driving quality improvements.
Salary and Benefits

The salary for this position is estimated at £42,000-£58,000 per annum, depending on experience, plus access to exclusive benefits, including:
