About the Job

As an Adult Social Care Specialist, you will be responsible for providing high-quality social work services to adults with physical disabilities, learning disabilities, mental health needs, and older adults in Hereford City, Ross on Wye and Leominster.

Key responsibilities will include conducting Care Act Assessments and support planning using a Strength Based approach, managing Complex Health Cases, undertaking Section 42 Enquiries, and implementing Community DoLS.

Our team provides support for adults with various needs, and we strive to provide a holistic service that prioritizes individual well-being.

About You
  1. You must be registered as a social worker with Social Work England.
  2. You should have awareness and understanding of current legislation and guidance and their impact on the adult social care profession.
  3. You should have knowledge of undertaking person-centred, strengths-based, outcome-focused assessment, care management, and review.
  4. You should have the ability to undertake Safeguarding Enquiries commensurate with training and post-qualifying experience.
