Job Title: Herefordshire Social Assessment Role

About the Role

We are seeking an experienced Social Assessor to join our team in Hereford. As a key member of our social care team, you will be responsible for undertaking strengths-based assessments, mental capacity assessments, care management, and reviews for adults with complex needs.

The ideal candidate will have a strong understanding of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, Human Rights Act 1998, Equality Act 2010, Mental Health Act 1983/2007, and Data Protection Act. You will also possess excellent communication and interpersonal skills, with the ability to work effectively with adults, informal carers, and other professionals.

This is a permanent role working 37 hours per week, with opportunities for training and supervision. The salary for this position is £38,000 - £42,000 per annum, depending on experience, plus generous holiday entitlement and access to a contributory pension scheme.

Key Responsibilities
  • Undertake strengths-based assessments, mental capacity assessments, care management, and reviews
  • Work with adults with complex needs, including those with learning difficulties/disabilities, mental health issues, and forensic needs
  • Develop and implement individualised care plans to meet the needs of service users
Requirements
  • NVQ3 qualification or above in a related field, or equivalent qualification/experience
  • Experience and an understanding of Adult Social Care, including statutory duties and principles
  • Awareness and understanding of current legislation, including the Care Act 2014 and Mental Capacity Act 2005
  • Ability to work as part of a wider team, with good communication and interpersonal skills
