Clinical Care Specialist

The Hampshire and Isle of Wight Integrated Care Board is seeking a skilled professional to join our All Age Continuing Care team as a Clinical Care Specialist. This dynamic role offers flexibility with hybrid working, allowing you to balance office and home-based work.
Main Responsibilities:
- Support individuals across all ages requiring continuing care by ensuring they receive the best possible support.
- Work collaboratively with hospitals, community services, and care providers to develop efficient systems and processes for assessment and referral under the supervision of case co-ordinators.
- Regularly review patients' clinical needs with regard to appropriateness of placement, level of care being provided, and entitlement to NHS Funded care, under the supervision of Senior Clinical Assessors.

Person Specification:
Knowledge, Training and ExperienceEssential- Registered Health or Social Care Professional with current registration with a professional body i.e. NMC, HCPC or SWE.
