Job Title: Clinical Care Coordinator

About the Role:
As a Clinical Care Coordinator, you will be responsible for ensuring the well-being of our residents in Sunnyview House. Your primary goal is to provide exceptional care, addressing their physical, emotional, and social needs.

Key Responsibilities:
* Take ownership of resident care, working closely with the care team to develop and implement individualized care plans.
* Monitor work areas and practices, ensuring they meet relevant standards, policies, and legislation.
* Collaborate with colleagues to deliver best practice, driving continuous improvement in clinical excellence.

Requirements:
* A valid NMC pin and previous experience as a Registered Nurse are essential.
* You must have excellent communication and leadership skills, with the ability to coach and guide the team.
* Strong attention to detail and organizational skills are necessary for this role.
