Lincolnshire Community Health Services NHS Trust is introducing a fully integrated Discharge to Assess Hub, reducing delays and delivering improved outcomes.

As a member of our 7-day Adult Care rota, you will work one weekend in four with scheduled shifts. This provides a predictable work-life balance.
