**Location:** Lancashire and South Cumbria NHS Foundation Trust

**Salary Range:** £51,075 - £76,087 per annum (dependent on experience)

**About the Role:**

We are seeking an experienced and skilled nurse to lead our community children's health service as a Community Children's Health Lead. This is an exciting opportunity for a professional with a passion for delivering high-quality patient care and making a positive impact on the lives of children and young people.

In this role, you will work closely with our multi-disciplinary team to develop and implement comprehensive care packages for children and young people with complex physical health needs. You will be responsible for assessing, planning, implementing, and evaluating individualised care plans, ensuring that each child receives the best possible outcomes.

You will have the opportunity to work collaboratively with other healthcare professionals, including doctors, therapists, and social workers, to develop and implement evidence-based care pathways. Your exceptional leadership and management skills will enable you to inspire and motivate your team, ensuring that they are equipped to deliver high-quality care to children and young people.

**Key Responsibilities:**
  • To lead the development and implementation of comprehensive care packages for children and young people with complex physical health needs.
  • To assess, plan, implement, and evaluate individualised care plans, ensuring that each child receives the best possible outcomes.
  • To work collaboratively with other healthcare professionals to develop and implement evidence-based care pathways.
  • To inspire and motivate the team, ensuring that they are equipped to deliver high-quality care to children and young people.
**Requirements:**

We are looking for an experienced and skilled nurse with:

  • A Registered Nurse qualification in Paediatrics.
  • A minimum of 2 years post-qualifying experience in Paediatric nursing.
  • Excellent leadership and management skills, with the ability to inspire and motivate others.
  • Exceptional communication and interpersonal skills, with the ability to build strong relationships with families, carers, and other stakeholders.
  • The ability to work collaboratively as part of a multi-disciplinary team, with a flexible and adaptable approach.
