Advanced Clinical Practitioner

We are seeking an experienced and skilled Advanced Clinical Practitioner to lead our Frailty Services team. As a key member of our multidisciplinary team, you will work collaboratively with healthcare professionals to deliver high-quality patient care.
Job DescriptionThis is a challenging yet rewarding role that requires strong clinical skills, leadership abilities, and a passion for improving patient outcomes. You will be responsible for:
- Leading the development and implementation of Frailty Services initiatives, in collaboration with senior leaders and clinicians.
- Providing expert clinical guidance and support to junior staff members.
- Collaborating with other departments to ensure seamless patient care transitions.
- Participating in quality improvement initiatives and contributing to the development of new policies and procedures.
To be successful in this role, you will need:
- Relevant MSc and specialist training experience.
- Independent non-medical prescriber qualification (where applicable).
- Strong leadership and communication skills.
- Able to work effectively in a fast-paced environment.
We offer a competitive salary of £58,000 - £69,000 per annum, depending on experience, as well as a range of benefits including:
