About Our Role
We are seeking an experienced Adult Mental Health Practitioner to join our Community Mental Health Team in Lincolnshire. As a key member of the team, you will provide high-quality clinical care to individuals with mental health needs, focusing on assessment, treatment planning, and comprehensive discharge planning.

This is an excellent opportunity to work in a dynamic and evolving service, co-designing care pathways with partners to meet the rehabilitation and recovery needs of individuals accessing our service. You will have the chance to develop your skills and expertise in person-centered care, risk assessments, and outcome measurement.
