Mental Health Nurse

We are seeking a highly skilled and experienced Community Psychiatric Nurse to join our Older Persons Community Health Team in Port Talbot. As a key member of our multi-disciplinary team, you will be responsible for providing high-quality care and support to patients living in the community, primarily over the age of 65, who are presenting with symptoms suggestive of an organic illness (dementia, cognitive impairment) and other functional illnesses including depression and anxiety.
Main Responsibilities- Engage with patients and their families to assess their needs and develop personalized care plans.
- Implement and evaluate care plans, including the formulation of risk assessments and ensuring involvement of the multi-disciplinary team.
- Work closely with the Consultant Psychiatrist, Social Services, and third sector and voluntary agencies to ensure patients receive the right care and interventions.
- Provide education and support to patients and their families to promote independence and well-being.

Requirements- Professional Qualification - RMN
- Experience in Community Nursing Older Persons
- Able to demonstrate clinical expertise for band 6
- Knowledge of clinical risk assessments
- Knowledge of Safeguarding Adults/Children including the Mental Health Act and Mental Capacity Act and Deprivation of Liberty Safeguards
