We are seeking a highly motivated and experienced Registered Mental Health Nurse to join our multidisciplinary team as a Care Coordinator. This is an exciting opportunity to work in a dynamic and innovative environment, providing high-quality community mental health services to individuals and their families.
Key Responsibilities- Assess, implement, and evaluate care and treatment plans for individuals on the caseload, working autonomously but under the indirect supervision of a Band 7 registered nurse/team manager.
- Work collaboratively with other health colleagues and colleagues from the Local Authority to deliver a supportive service for those under the care of the CMHT.
- Provide mental health assessments and serve as a point of contact for existing service users in the absence of their care coordinator.
- Support clinic nurses during times of leave, participating in various clinics, including depot, Clozapine, Lithium, and physical health screening clinics.
- Registered Mental Health Nurse with a postgraduate qualification (diploma, degree, or higher qualification) or evidence of working towards the same.
- Experience of working at Band 5 within a multidisciplinary team framework in Acute Adult Mental Health Services.
- Experience of joint working with other agencies/services.
- Ability to work autonomously, communicate effectively with service users, and travel between sites in a timely manner.
- Knowledge of evidence-based practices within a mental health setting, Health Board Policies and Procedures, medication, risk assessment, and clinical audit/governance.
- Relevant Nursing Degree or Diploma in relevant Nursing/Community Studies.
- Experience of working with family and carers.
- Ability to use a word processor and database.
- Ability to speak Welsh.
