Job Title: Advanced Fostering Practitioner

This is an exciting opportunity for a highly skilled and experienced healthcare professional to join our team as an Advanced Fostering Practitioner. As a key member of our Acute Medicine department, you will play a vital role in providing high-quality patient care and contributing to the development of our team.

Job Summary:

The successful candidate will be responsible for coordinating the care of acute medical patients throughout the care pathway, working closely with the multidisciplinary team and clinical services to ensure effective management of this group of patients. You will use advanced clinical assessment skills under clinical supervision to manage patients with complex needs, and develop treatment plans for patients requiring urgent intervention.

Key Responsibilities:
  • Co-ordinate the care of acute medical patients throughout the care pathway.
  • Use advanced clinical assessment skills to manage patients with complex needs.
  • Develop treatment plans for patients requiring urgent intervention.
  • Communicate effectively with patients, carers, and the multidisciplinary team.
  • Act as an advocate for patients and colleagues.
Requirements:
  • Highly developed communication skills.
  • Ability to work under clinical supervision.
  • Experience in acute medicine or a related field.
  • Advanced clinical assessment skills.
  • Ability to develop treatment plans for patients requiring urgent intervention.
