Job Summary

We are seeking a highly experienced and knowledgeable Clinical Nurse Specialist to join our Paediatric Neurology team. The successful candidate will work closely with the multidisciplinary team to provide comprehensive follow-up and support to patients with neurology conditions, including epilepsy.

Main Duties

The post holder will develop and provide an advisory and supportive service to patients under the Paediatric Neurology service, working primarily with patients who have a known or suspected diagnosis of epilepsy. They will provide education and information to children, young people, and their families regarding the management and treatment of complex seizures.

The Clinical Nurse Specialist will work to develop the tertiary neurology service at Chelsea and Westminster NHS Foundation Trust, expanding its portfolio of treatments offered to patients with complex seizure presentations. They will provide patient follow-up, developing a structured pathway and monitoring email and telephone contacts, liaising effectively with all members of the multidisciplinary team.

The post holder will maintain oversight of the service's activity, governance, and clinical outcomes, working with the team on change initiatives as required. They will act as an expert resource to professionals involved in the care of these patients, both within the acute and primary care setting.

Key Responsibilities
  • To receive referrals within the specialty from the healthcare team and respond to requests for expert advice and guidance.
  • To work within an agreed job plan to meet service needs, which should be reviewed annually.
  • To conduct holistic assessments and provide supervision and guidance to colleagues in skilled history taking, physical examination, observation, and ordering, conducting, and interpreting relevant tests and investigations.
  • To work in collaboration with the multi-professional team, contributing expert, evidence-based knowledge and skills to relevant aspects of patient care and treatment.
  • To maintain boundaries of own practice within agreed guidelines and protocols, use professional judgment in managing complex and unpredictable situations, and seek guidance from other relevant experts.
  • To address patient risk factors and health promotion needs through comprehensive assessment of individuals for risk factors and early signs of illness.
  • To build an environment of trust and rapport, enabling patient empowerment in their healthcare and acting as an advocate for the patient in treatment decisions and choices.
  • To draw upon an appropriate range of multi-agency and inter-professional resources to ensure that patients receive appropriate services, making referrals to other professions and services as appropriate.
  • To use effective communication and interpersonal skills to provide information, explanation, and reassurance to patients and families.
  • To plan and conduct appropriate follow-up, closure, or transition of care for the patient.
  • To maintain accurate and contemporaneous records of patient assessment, care, and treatment.
