Clinical Pharmacist

We are seeking a highly motivated and experienced Clinical Pharmacist to join our Medicines Management team at Central London Community Healthcare Trust. As a key member of our team, you will play a vital role in ensuring the safe and effective management of medicines across our services.
Key Responsibilities- Provide expert advice and guidance on medicines management and governance to healthcare professionals across the Trust.
- Contribute to the development and implementation of policies and procedures related to medicines management.
- Lead and manage a team of pharmacists and other healthcare professionals to deliver high-quality clinical pharmacy services and medicines optimisation initiatives.
- Collaborate with other stakeholders to ensure seamless integration of medicines management across our services.
- Develop and maintain strong relationships with healthcare professionals, patients, and carers to ensure effective communication and delivery of medicines management services.
- Vocational Masters Degree in Pharmacy.
- Professional registration with the General Pharmaceutical Council (GPhC).
- Postgraduate Clinical Pharmacy Certificate qualification or equivalent.
- Postgraduate Clinical Pharmacy Diploma qualification or equivalent.
- Continuing Professional Development (CPD) portfolio demonstrating personal responsibility for ongoing learning and professional development.
- Independent Prescriber or working towards qualification.
- Post-qualification experience of clinical pharmacy, medicines information, and prescribing advice.
- Experience of working in secondary care and in a multidisciplinary environment.
- Experience of providing a Clinical Pharmacy Service with good medicines management initiatives.
- Experience of working effectively within multi-disciplinary groups.
- Experience of undertaking structured medicine reviews.
- Experience of training and tutoring in a hospital setting.
- Experience of audit techniques.
- Experience in supervising a team/service.
- Sound knowledge of prescribing, pharmacy, and medicines management issues faced in primary and secondary care.
- Up-to-date clinical and pharmaceutical knowledge.
- Excellent communication and interpersonal skills.
- Effective negotiation and persuasion skills.
- Ability to provide succinct and objective advice.
- Good problem-solving skills.
- Team player with ability to work independently.
- Ability to adapt to different working environments.
