Job Summary:

We are seeking a skilled and compassionate Health Care Assistant to join our Community Care team at Demelza. As a Health Care Assistant, you will play a vital role in delivering high-quality care and support to children and young people with complex health needs.

Key Responsibilities:

  • Provide person-centered care and support to children and young people, promoting their physical, emotional, and social well-being.
  • Work collaboratively with multidisciplinary teams, including healthcare professionals, social workers, and families, to develop and implement individualized care plans.
  • Conduct regular assessments and reviews to ensure the ongoing needs of children and young people are met.
  • Develop and maintain strong relationships with families, carers, and other stakeholders to ensure seamless care coordination.
  • Participate in training and professional development opportunities to enhance your skills and knowledge in health and care.

Requirements:

  • Level 2 qualification in a health or care-related subject, or a willingness to complete.
  • Experience of working with children and young people with complex health needs.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups.
  • Ability to work flexibly, including evenings and weekends, and travel across our operating region.
  • Full driving license and access to a vehicle for business use, or reliable transport arrangements.
