This exciting opportunity offers a chance to work as a Healthcare Transformation Specialist, playing a key role in developing and implementing strategies to improve healthcare outcomes in North Central London.


Job Description


The successful candidate will be responsible for working closely with the NCL Training Hub team to deliver a second-year long-term conditions locally commissioner service (LCS) support programme.


Main Responsibilities


The post holder will:



  • Proactively develop relationships with stakeholders, including Primary Care Networks, general practices, and other practitioners to drive improvements in business processes.
  • Explain and embed approaches to support proactive management of practice lists to promote personalisation approaches being introduced across all general practice.
  • Promote the Long-Term Condition Service to ensure comprehensive support from all practices and PCNs is in place.
  • Provide guidance to ensure that business change activities are aligned to supporting improvements in clinical practice.
  • Act as the senior manager to coordinate the programme and activities related to the LTC LCS for the borough.

About the Training Hub


The North Central London (NCL) Training Hub is a network of multidisciplinary primary care teams, community providers, and social care providers. The hub receives core funding from Health Education England but also has extra commissions from NHS England & Improvement and Local Authorities.


LTC Specific Responsibilities


The post holder will:



  • Work with a broad range of internal and external stakeholders, including North Central London Training Hub leads, ICS Primary Care teams, Primary Care Networks (PCNs), and General Practices.
  • Develop and maintain excellent relationships with practices and other providers to ensure continuous development of new career pathways, development, and implementation of new ways of working and relevant recruitment and retention strategies.
  • Participate in relevant internal and external working groups/projects, services, and initiatives to provide information and analytical advice and expertise.
  • Develop and implement data collection systems that will provide accurate and timely data.
  • Present information and issues, explaining highly complex issues, to a wide range of internal and external stakeholders.

Project Management


The post holder will support the development of project plans, identifying risks, issues, and dependencies, considering best practice and current options.


Financial and Physical Resources


The post holder will act in a way that is compliant with standing Orders and Standing Financial Instructions in the discharge of budget management responsibilities.


Staff Management


The post holder will forge close positive working relationships to support effective matrix working.


Information Management


The post holder will draft reports summarising issues, appraising outcomes, and providing progress reports for the programme managers and programme management meetings.


Planning and Organisation


The post holder will contribute to project planning by identifying interdependencies across projects / workstreams, including potential impacts on wider organisational delivery.


Salary


We offer a competitive salary of approximately £60,000 per annum, commensurate with experience and qualifications.
