Key Responsibilities

As a Nurse Assessor for Complex Needs, you will be responsible for:

Conducting comprehensive assessments of patient care needs

Co-producing individualised care and support plans to meet the complex needs of patients

Coordinating the process of sourcing appropriate care provision

£52,274 - £66,306 per annum (dependent on experience)

You will be part of a strong, supportive, high-performing team focused on quality, making recommendations for CHC eligibility, and supporting the implementation of the national framework.
