Job Title: Family and Networks Practitioner

We are seeking a highly skilled and motivated General Practitioner to join our dynamic Primary Care Network (PCN) team. As a Family and Networks Practitioner, you will play a vital role in delivering high-quality patient care and contributing to the development of our local services.

Key Responsibilities:
  • Provide autonomous general practitioner services, working independently and collaboratively with other healthcare professionals to assess, diagnose, and treat patients in a primary care setting.
  • Offer expert professional advice to patients, carers, and colleagues, ensuring the maintenance of clinical excellence.
  • Participate in the provision of education and training for GP Registrars and other staff.
  • Contribute to the development of care plans for patients, in consultation with them and in line with current practice disease management protocols.
  • Collect data for audit purposes and compile and issue computer-generated acute and repeat prescriptions.
  • Perform physical examinations and/or preventive health measures to assess and evaluate patients' clinical problems and healthcare needs.
  • Undertake daily minor illness/injury clinics and promote primary and secondary prevention.
  • Accurately and timely input patient data onto the clinical system.
  • Lead and support within a team of different healthcare professionals and staff.
Requirements:
  • Newly Qualified General Practitioner (with under 2 years from qualification).
  • General understanding of the primary care contract.
  • Excellent communication skills (written and oral), strong IT skills, clear, polite telephone manner, competent in the use of Office and Outlook, EMIS user skills, effective time management (planning and organizing), ability to work as a team member and autonomously, excellent interpersonal skills, problem-solving and analytical skills, ability to follow clinical policy and procedure, experience with audit and able to lead audit programmes, experience with clinical risk management.

Our PCN is a Primary care Network of all 5 Bromley practices working collaboratively to enhance the health and wellbeing of Bromley residents, covering over 40,000 patients, 100% of the Bromley population.

Employment will be hosted by 1 of the 5 member practices within the PCN for payroll and HR purposes.
